Congregational Home admits patients from all local hospital systems, including Froedtert Health, Prohealth, Ascension & Aurora.  We are a Preferred Provider with most of these hospital systems, including membership in their Accountable Care Organizations, Medicare 3-Day Waiver Programs and access to their electronic medical records systems.

 In addition, Congregational Home participates in Froedtert Health and Prohealth Preferred Provider partner meetings to improve patient outcomes and ensure continuity of care and seamless transitions with our mutual patients.

 Froedtert Health has also placed a Froedtert Mobile Care Team at Congregational Home, making onsite primary care with a physician and/or nurse practitioner available to our short-term rehab, assisted living, nursing home & hospice patients, Monday through Friday.  For primary care during off hours on nights, weekends & holidays, we offer physician telehealth video visits.
