Congregational Home is a non-profit, faith-based Continuing Care Retirement Community (CCRC), otherwise known as a Life Plan Community, offering a Rare Full Continuum of Care for seniors: Short-Term Subacute Rehabilitation, Independent Living, Assisted Living (RCAC & CBRF), Memory Care, Skilled Nursing Home Care and Hospice Care – all under one roof. We are one of a small percentage of organizations in the region with this full continuum and the only one in Brookfield.
In addition, Congregational Home’s Rare Dual License allows Independent Living & Assisted Living (RCAC) in the same apartment. WITHOUT this dual license, Independent & Assisted Living would have to be provided in separate areas, causing a couple with two different levels of care to be split up or an individual to have to move when they transition from Independent to Assisted Living.

Founded by the First Congregational Church of Wauwatosa, Wisconsin, Congregational Home has been a pillar of the Brookfield community since 1974, and welcomes all faiths. Unlike huge complexes & soaring towers, our smaller setting gives a feel of family & allows for more personalized attention. With our non-profit status, we return our profits back into our organization for the betterment of those we serve, enabling us to attract higher-qualified staff & maintain one of the highest nursing staff levels in the state.
Congregational Home has MEDICARE’S Highest 5-Star Rating for Quality Measures and we are proud to be named a 2024 High Performing Short-Term Rehab Facility by US NEWS & WORLD REPORT and the 2024 Top Choice for Nursing Home Care by MILWAUKEE JOURNAL SENTINEL readers.
